CASE MANAGEMENT
Case management services help identify persons who are eligible for the Elderly Waiver Program. Once eligible, those clients are assisted to access such services as personal emergency response systems, home delivered meals and transportation.
Clients may also qualify for Chore services and get assistance with snow removal, lawn mowing and heavy house cleaning. Homemaker services may also be a benefit, providing assistance with routine housekeeping, grocery shopping, laundry and meal preparation.
